Introducing Century Aesthetics Rewards

At Century Aesthetics, Dr. Fodor wants his patients and medi-spa clients to have it all and more. As of October 2009, all current, former and new patients and spa clients will be automatically enrolled in the new Century Aesthetics Rewards Program. Our Rewards Program offers two components:

Patient/Spa Client Procedure or Treatment Rewards

All new, current and past patients of Dr. Fodor's will be enrolled in the program with an automatic reward (valued at $250) that can be applied to the cost of any new surgical procedure, injection or spa treatment booked as of November 2009. Spa Clients will also receive a reward (valued at $50) that can be used towards the cost of any treatment booked (at $100 value or more) when they book a new appointment as of November 2009. This will be a one time, automatic reward (also see Referral Rewards).

Then each time you pay for a procedure or treatment within the Century Aesthetics practice or Century Cosmetics Medi-spa, you will earn "rewards credit" towards future procedures and treatments from injectables to surgery. The values of the rewards are based upon type of procedure purchased. A rewards value list is available through the front desk.

Procedure and Treatment Rewards are transferable to friends, family and other patients upon request who are new or current patients or spa clients (also see Referral Rewards) as long as the person is determined to qualify for that procedure.

Patient/Spa Client Referral Rewards

When you refer a new patient or client who has a surgical procedure with Century Aesthetics you'll receive an automatic $250 Referral Reward. At the same time, that new patient and or client will also receive an automatic reward of the same value.

Automatic Referral Rewards can be used towards new procedures and treatments booked and can also be transferred to another patient or spa client, again as long as the person is determined to qualify for that procedure.

• Build and use your Referral Rewards a little at a time or all at once towards the purchase of procedures, treatments and products.
• All Rewards earned are completely transferable to current or new patients or (spa) clients as long as that person is determined to qualify for that procedure.
• In order to receive your Referral Rewards, everyone you refer must mention your name (as a current or past patient) when making their first appointment.
• We will keep track of your Rewards and you will receive a quarterly report so that you can keep track. If you ever want to transfer your Rewards to friend, family member or another patient just call the office and we will take care of it. You will then receive a receipt of acknowledgement that you will sign and return to the office in order for the transfer to be completed. We can do this transaction if email or regular mail. We ask that you keep your current email on file with us for easy communication.
